A Secretaria Municipal de Saúde de Nova Serrana informa que já está disponível novo agendamento para vacinação contra Covid-19, para pessoas a partir de 35 anos de idade, residentes em Nova Serrana. O agendamento será feito pelo site da prefeitura (www.novaserrana.mg.gov.br) apenas até o preenchimento das 3.300 vagas.
Documentos necessários: Identidade; CPF; Comprovante de endereço; Cartão SUS; Cartão de vacina. A vacinação ocorrerá entre terça (27) e quarta-feira (04/8), com exceção do final de semana e de segunda (2/2).
– Orientações gerais:
Quem não tiver acesso à internet ou tiver dificuldade de agendamento devem procurar a unidade de saúde de referência ou fazer contato através dos telefones: 3225-4075 / 99971-6345.
Agendamento fora dos critérios do Plano Nacional de Imunizações, sem documentos comprobatórios e/ou fora da faixa etária, serão devidamente CANCELADOS pela equipe de vacinação.
Não ter recebido qualquer outra vacina nos últimos 14 dias;
Não ter tido Covid-19 com início de sintomas nos últimos 30 dias
Além disso, o agendamento será enviado aos órgãos de controle externo e, em caso de informações inverídicas, ficarão sujeitos às responsabilizações administrativas, civis e penais aplicáveis.
