GARANTA GRATUITAMENTE CARTEIRA DE IDENTIFICAÇÃO DA PESSOA COM TRANSTORNO DO ESPECTRO AUTISTA

Desde 2020 o município de Nova Serrana emite de forma gratuita a Carteira de Identificação da Pessoa com Transtorno do Espectro Autista (Ciptea) Municipal, conforme Decreto 014/2020 e Lei Federal Nº 13.977 de 2020. Para fazer a solicitação é necessário providenciar os seguintes documentos:

Formulário de requerimento (em anexo no site hwww.novaserrana.mg.gov.br);

Atestado Médico de especialista em psiquiatria ou neurologia, com diagnóstico do Transtorno do Espectro Autista e Classificação Estatística Internacional de Doenças e Problemas Relacionados com a Saúde – CID;

Comprovante de tipo sanguíneo;

Carteira de Identidade (do usuário e dos pais ou representante legal);

CPF (do usuário e dos pais ou representante legal);

Comprovante de endereço (cópia);

Duas fotos 3×4 do requerente e uma do responsável;

Nos casos em que a pessoa com transtorno do espectro autista seja imigrante detentor de visto temporário ou de autorização de residência, residente fronteiriço ou solicitante de refúgio, deverá ser apresentada a Cédula de Identidade de Estrangeiro (CIE), a Carteira de Registro Nacional Migratório (CRNM) ou o Documento Provisório de Registro Nacional Migratório (DPRNM), com validade em todo o território nacional.

Para a realização do exame tipagem sanguínea é necessário fazer o agendamento na Unidade de Saúde de Referência. Procure o PSF com documento que comprove o Transtorno do Espectro Autista e solicite o agendamento na recepção.

Após providenciar todos os documentos basta levar até a Secretaria Municipal de Desenvolvimento Social, que fica na rua Antônio Martins, nº 50, bairro Park Dona Gumercinda Martins.
